Özet
Uzun süre hipertansif kalan kişilerde ortaya çıkabilen bir komplikasyon da Martorell sendromudur. Hastamız 48 yaşında obez, 6 aydır sağ bacağının 1/3 distalinde anteromedial yerleşimli enfekte ve 6 aydır tedavi görmüş ancak yapılan düzenli pansumanlara rağmen ülserde iyileşme görülememiş bir bayandı. Hasta yatırıldı ve kan basıncı 230/130 mmHg bulunduğu için antihipertansif ve antibiyotik tedavisine alındı. Pansumanların aşırı ağrılı olması nedeniyle epidural anestezi kateteri yerleştirildi. 2 haftalık debridman ve pansuman sonrası hasta ameliyata alınarak split thickness deri grefti ile ülser kapatıldı. Spontan ya da travma sonucu açılan alt ekstremite ülserlerinde kan basıncının ölçümü, hastada tüm tedaviyi değiştirebilecek bir bulgu olabilir.
Martorell's Syndrome is one of the complications that result from Long lasting uncontrolled hypertension. Our case was a 48 years old obese female who had been suffering from an infected spontaneous leg ulcer which was localized anteromedially in the 1/3 distal part of the right leg. In spite of appropriate surgical wound therapy for 6 months, the ulcer was not healed. She has not known her hypertension until she came to our center. She was hospitalized and antihypertensive therapy was added to the management when the blood pressure was found to be 230/130 mmHg. For the problem of pain during debridements, an epidural catheter was inserted. After debridement and appropriate antibiotherapy for two weeks infection was over and she was operated to cover the ulcer with a split thickness skin graft. Checking blood pressure in a patient with traumatic or spontaneous leg ulcer may affect the therapeutic decision making.
