Özet
Bu araştırma, göğüs cerrahisi geçiren hastaların öz yeterlilik düzeylerinin mobilizasyon süreci üzerindeki etkilerini belirlemeyi amaçlamaktadır. Çalışma, tanımlayıcı ve kesitsel bir tasarımla gerçekleştirilmiştir. Araştırmanın evreni, göğüs cerrahisi servisinde yatan ve operasyon geçiren hastaları kapsamaktadır. Örneklem olarak, güç analizi kullanılarak yeterli belirlenene sayı 123’tür. Ancak araştırma Bakırçay Üniversitesi Çiğli Eğitim ve Araştırma Hastanesi'nde yatan 132 hasta ile yürütülmüştür. Araştırmanın amacına uygun olarak hastaların öz yeterlilik düzeyleri “Genelleştirilmiş Özyeterlilik Ölçeği” ile mobilizasyon seviyeleri “Hareketlilik Ölçeği” ile değerlendirilmiştir. Veri toplama süreci, İstanbul T.C. Maltepe Üniversitesi Araştırma Etik Kurulu'ndan yazılı izin alınarak ve hastaların bilgilendirilmiş onamları alınarak başlatılmıştır. Veriler, yüz yüze görüşmeler ve ölçekler aracılığıyla toplanmıştır. Analizler sonucunda, öz yeterlilik düzeyinin mobilizasyon üzerindeki etkileri IBM SPSS Statistics paket program ile değerlendirilmiş ve ilişkinin doğası incelenmiştir. Araştırma bulguları, öz yeterlilik düzeyinin mobilizasyon süreci üzerinde önemli bir rol oynadığını göstermiştir. Yaş ve tütün-alkol kullanımı Hasta Hareketlilik Ölçeği(HHO) üzerinde etkili saptanamamıştır. Cinsiyet ve medeni durum ölçek puanlarını etkilememiştir. Genç yetişkinler (18-34 yaş) diğer yaş gruplarından, aktif çalışanlar çalışmayanlardan, tütün-alkol kullananlar kullanmayanlardan daha yüksek Genelleştirilmiş Özyeterlilik Ölçeği(GOY) puanlarına sahiptir. Eğitim düzeyi yüksek olanlar hem HHO hem de GOY puanlarında diğer gruplardan daha yüksek skorlar elde etmiştir. Gelir seviyesi düşük olanlar, HHO puanlarında diğer gruplardan daha yüksek skorlar elde etti; ancak GOY puanları daha yüksek gelir seviyesine sahip olanlarda daha yüksektir. Hastanede yatma deneyimi olanlar, HHO puanlarında daha yüksek skorlar elde etmiş ancak GOY puanları yatmayanlarda daha yüksek saptanmıştır. Ek kronik hastalığı olanlar, HHO puanlarında daha yüksek skorlar ederken GOY puanları ek kronik hastalığı olmayanlarda daha yüksek bulgulanmıştır. HHO ile GOY arasında: orta düzeyde negatif bir ilişki bulunmuştur (r=0,238, p<0,01). Bu nedenle hasta hareketlilik ölçeği puanları arttıkça genelleştirilmiş özyeterlilik ölçeği puanları azalmaktadır. Sonuç olarak, araştırma göğüs cerrahisi geçiren hastaların mobilizasyon süreçlerinde öz yeterlilik düzeyinin kritik bir faktör olduğunu ortaya koymuştur. Bu bulgular hem sağlık profesyonelleri hem de hasta yönetimi açısından önemli uygulama ve yönlendirmeler sağlamaktadır.
This study aims to determine the effects of self-efficacy levels of patients undergoing thoracic surgery on the mobilization process. The study was conducted with a descriptive and cross-sectional design. The population of the study included patients hospitalized in the thoracic surgery service and undergoing surgery. As a sample, the adequate number determined by using power analysis was 123. However, the study was conducted with 132 patients hospitalized in Bakırçay University Çiğli Training and Research Hospital. In accordance with the purpose of the study, the self-efficacy levels of the patients were evaluated with the Generalized Self-Efficacy Scale and their mobilization levels were evaluated with the Mobility Scale. The data collection process was initiated by obtaining written permission from the Istanbul T.C. Maltepe University Research Ethics Committee and informed consent of the patients. Data were collected through face-to-face interviews and scales. As a result of the analysis, the effects of self-efficacy level on mobilization were evaluated with IBM SPSS Statistics package program and the nature of the relationship was examined. The findings of the study showed that, in general, self-efficacy level played an important role on the mobilization process. Age and tobacco-alcohol use were not found to be effective on HRQOL. Gender and marital status did not affect scale scores. Young adults (18-34 years old) had higher GHI scores than other age groups, active workers than non-workers, and tobacco-alcohol users than non-users. Those with higher levels of education had higher scores than other groups in both HHO and GOY scores. Those with lower income levels scored higher than other groups in HHO scores; however, GOY scores were higher in those with higher income levels. Those with hospitalization experience scored higher on the HHO scores, but the GOY scores were higher in those without hospitalization. Those with additional chronic diseases had higher scores in HHO scores, whereas GOY scores were higher in those without additional chronic diseases. A moderate negative correlation was found between the HHO and the GOY (r=-0.238, p<0.01). Therefore, as patient mobility scale scores increase, generalized self-efficacy scale scores decrease. In conclusion, the study revealed that self-efficacy level is a critical factor in the mobilization process of patients undergoing thoracic surgery. These findings provide important applications and directions for both healthcare professionals and patient management.
