Özet
Epispadiaslı ve ekstrofilerde idrar kontinansını sağlamak en zor başarılan amaçlardan biridir. Bu problemin çözümü için mesane boynu rekonstruksiyonuna alternatif olarak artifisyel sfinkter önerilmektedir. Kliniğimizde 25 yaşındaki bir erkek hastaya tek parçalı ‘Flow Secure artifisyel sfinkter' yerleştirdik. Epispadias ve ekstrofi ile doğmuş olan hasta bize gelene kadar mesane boynu rekonstrüksiyonu da dahil sekiz operasyon geçirmişti. Total inkontinans şikayeti mevcuttu. Ürodinami ve sistoskopiyle mesane kapasitesi ve fonksiyonunun normal olduğu görüldü. Üretrada darlık izlenmedi. Flow secure tek parçalı sfinkter hastaya takıldı. Komplikasyonsuz taburcu edilen hastanın 6 hafta sonra sfinkter aktivasyonu yapıldı. Tam kontinans sağlandı.
Urinary continence is one of the most difficult goals to achieve in patients with exstrophy or epispadias. The artificial genitourinary sphincter provides an alternative to reconstruction of the bladder neck for management of this problem. We implanted the artificial urinary sphincter in 25 years old male patient. He was born with epispadias-exstrophy complex. Although he had many operations including bladder neck reconstruction, the patient admitted to our clinic with total incontinence. We evaluated the patient with cycstoscopy and urodynamic test before sphincter implantation. Bladder capacity and function were normal. Urethal athrophy with no stricture was detected. We implanted Flow Secure single part artificial sphincter. We did not see any complication after operation. Urethral cathater was removed post operative third day. We activated the sphincter six weeks later. Total continence was achived.
