Bilateral Mutifocal Intraocular Lens Implantation on an Postoperative Hyperopic Patient

Purpose: Presentation of our bilateral multifocal intraocular lens (IOL) implantation results on an bilateral postoperative hyperopic laser in situ keratomileusis (LASIK) patient. Material-Methods: In April 2007, a female, 10 year- postoperative bilateral hyperopic LASIK patient was implanted by the same surgeon with bilateral multifocal IOLs following a facoemulsification operation, because she had developed cataract. Patients preoperative and postoperative visual acuities (distance, near and intermediate on both Snellen and Jaeger charts), corneal topography, keratometry values and postoperative contrast sensitivity results were followed up. Power of the IOL was calculated according to clinical narrative method. Patient was also asked questions regarding her quality of life, light reflections and personal satisfaction. Results: 70 year old female subject had received hyperopic LASIK treatment 10 years ago (OD: +3.75 D; OS: +3.50 -0.50 90 D). After cataract has developed, in April 2007, patient received bilateral multifocal IOL implantation following facoemulsification surgery. Right eye received a refractive IOL, while left eye received an aspheric diffractive IOL. At 2 months postoperative, uncorrected visual acuities were as follows; distant 0.8 and 1.0, near J3 and J1, intermediate J5 and J3. Postoperative best corrected visual acuities were as follows; distant 1.0 and 1.0, near J1 and J1, intermediate J2 and J1. At 2 months, postoperative refraction values were -0.75 D for the right eye and +0.00 for the left eye. Contrast sensitivity tests were conducted at 4th, 6th months and 1 year postop and improvement was observed. Patient noted the glare problems were diminishing by day. Overall satisfaction was rated as good. Conclusion: Our bilateral hyperopic LASIK patient was satisfied following the bilateral multifocal IOL implantation. Although the result is encouraging, further studies on more subjects are needed for proving the effectiveness and safety of the technique.
