Özet
AMAÇ: Miyokardiyal iskemi/reperfüzyon hasarına pentoksifilin ile zenginleştirilmiş St.Thomas II solüsyonunun etkisi mongren köpeğinde araştırıldı. GEREÇ VE YÖNTEM: Denekler, her biri 10 denek içeren iki gruba ayrıldı. Çalışma grubuna, 3 mg/kg pentoksifilin, St.Thomas II kardiyoplejik solüsyonu içersine ilave edildi. Kontrol grubunda ise sadece klasik St. Thomas II solüsyonu kullanıldı. Her iki grupta LAD oklüde edildi ve 60 dakikalık iskemi periodundan sonra kardiyo-pulmoner bypass'a girildi ve kardiyopleji verilerek hipoksik arrest uygulandı. LAD'deki oklüzyon kaldırıldı. BULGULAR: Kardiyopulmoner bypasstan çıkış döneminde kardiyak indeks (CI) çalışma grubunda 3.5±0.4 kontrol grubunda 2.6+0.3 olarak bulundu. Reperfüzyon periyodunda kontrol grubunda CI 1,3,5 saatlerde 2.7±0.4, 3.4±0.5, 3.5±0.7 lt/m2 iken çalışma grubunda aynı değerler 3.1±0.4, 3.8±1.2, 3.9±1.1 lt/m2 olarak saptandı (p<0.05). Kontrol grubunda üç hastaya inotropik destek gerekirken, çalışma grubunda hiç bir denekte inotropi ihtiyacı duyulmadı. Ultrastrüktürel incelemelerde kontrol grubunda Kloner skorlaması 3 iken çalışma grubunda 1 olarak değerlendirildi. SONUÇ: Pentoksifilin 5'-Nükleotidaz enzimini bloke ederek mitokondriyal alanda adenozin kaybını engeller ve miyokardın reperfüzyon hasarını anlamlı ölçüde azaltır.
AIM: The influence of pentoxifilin on ischemia/reperfusion damage was studied in the heart of Mongren dogs. MATERIALS AND METHODS: Dogs were divided into two groups and each groups were included 10 dogs. Pentoxifilin 3mg/kg was added into the cardioplejic solution. LAD was occluded by the snare in both groups and Cardiopulmonary bypass was initialed after 60 minutes ischemic period. Hearts were arrested with cardioplegia. LAD occlusion was removed and crossclamp released. RESULTS: Average cardiac index (CI) in just after cardiopulmonary bypass period were 3.5 ±0.4 lt/m2 in group I, 2.6 ±0.3 lt/m2 in group II. Average cardiac index in reperfusion period were measured in 1,3,5 hours. Average Cardiac index in group II in same intervals as follows, 2.7 ±0.4, 3.4±0.5, 3.5±0.7 lt/m2 but in Group I 3.1 ±0.4, 3.8±1.2, 3.9±1.1 (p<0.05). Three hearts in control and one in study groups were required inotropy. Ultrastructural examination by Kloner's scoring was 1 in group I and 3 in group II CONCLUSION: Pentoxifilin enriched St.Thomas II solution preserves adenosine in the mitochondrial space and reduces the reperfusion injury significantly.
