Yayın Glutamine enhances the heat shock protein 70 expression as a cardioprotective mechanism in left heart tissues in the presence of diabetes mellitus(Taylor and Francis Online, 2010) Uğurlucan, Murat; Erer, Dilek; Karatepe, Oğuzhan; Ziyade, Sedat; Haholu, Aptullah; Güngör Uğurlucan, Funda; Tireli, Emin; Dayıoğlu, Enver; Alpagut, Ufuk İbrahimObjective: Effects of diabetes mellitus on myocardium were investigated, by assessing levels of heat shock protein (HSP) 70, and efficacy of glutamine was tested. Materials and Methods: Thirty male rats were divided into three groups: control group (Group 1), diabetic group (Group 2) and glutamine-induced diabetic group (Group 3). Diabetes was created by intravenous streptozocin injection. Rats were examined one month later for cardiac complications of diabetes. Serum and tissue samples were obtained to measure HSP 70 levels. Results: Following streptozocin administration, glucose levels increased markedly. This resulted in a significant increase in HSP 70 in serum and tissues. When Group 3 was compared with other groups, HSP 70 was more increased in serum and tissues. When Groups 2 and 3 were compared, more increased HSP 70 values were observed in Group 3, statistical significance was obtained for left atrial and left ventricular HSP 70 levels. Elevated blood glucose was correlated with elevated HSP 70 levels. Increased serum HSP 70 levels were correlated with tissue HSP 70 values. Conclusions: HSP 70 levels increase in the myocardium of rats in diabetes mellitus as a protective mechanism. Levels of HSP 70 may further be increased with parenteral administration of glutamine. In this report, we present a case of lacrimal gland adenoid cystic carcinoma who treated by multimodal approach with intraarterial chemotherapy. A 32-year-old woman was diagnosed with adenoid cystic carcinoma (solid variant) of the left lacrimal gland. Neoadjuvant intraarterial cisplatin, intravenous doxorubicin, and orbital exenteration were performed. The patient was followed for 32 months without relapse. Information about citric acid intoxication is limited. Its ingestion is a life-threatening condition that requires a multidisciplinary approach.Case report: A 44 year-old male patient arrived to the hospital after a suicide attempt by taking 150 ml of dishwasher polisher. Vomiting was induced by his relatives before coming to hospital 4 hours after. Arterial blood-gas and electrolytes evaluation revealed blood-pH and potassium to be 7.15 and 6.13 mg/dL, respectively, and an increased plasma anion gap. Two weeks later, bilateral vocal cord paralysis was diagnosed and tracheostomy was performed. Internal Medicine consultation revealed a initial diagnosis of vasculitis and he was admitted to Internal Medicine Department. There was a diffuse cutaneous petechial rash which was nonpalpable and the largest one was about 2 cm diameter. The pathological punch biopsy sample taken from the lower part of the left leg, where there was diffuse rash, revealed leucocytoclastic vasculitis. One mg/kg prednizolone was started after vasculitis was confirmed pathologically. Cutaneous lesions recovered dramatically. Discussion: Many drugs are indicated to cause leucocytoclastic vasculitis. Antibiotic, especially those of penicilin group and clarithromycin are reported to cause leucocytoclastic vasculitis and henoch-schönlein purpura. Other agents that may cause leucocytoclastic vasculitis development are non-steroid anti-inflammatory drugs, proplytiouracil, paracetamol, simetidin, streptokinase, metformin and acenocumaral. There’s no other case in literature as ours who took foreign substances such as citric acid for suicidal purposes rather than treatment purposes. The only case found in literature about citric acid had metabolic acidosis with high level of anion gap, which recovered after ionized calcium infusion; however, its follow up do not report a leucocytoclastic vasculitis similar to our case.Yayın Nephrotic syndrome and acute kidney injury following CoronaVac anti-SARS-CoV-2 vaccine(Oxford Univ Press, 2021) Ünver, Suat; Haholu, Aptullah; Yıldırım, SukruA 67-year-old female with Type 2 diabetes mellitus developed nephrotic syndrome within 1 week of receiving the first dose of severe acute respiratory syndrome coronavirus 2 CoronaVac vaccine. A kidney biopsy was consistent with minimal change nephrotic syndrome and treatment was symptomatic with antiproteinuric therapy and improvement in proteinuria. Oedema returned within 1 week of the second dose of CoronaVac. On this occasion, acute kidney injury and massive proteinuria were noted. In kidney biopsy, glomeruli were normal, but tubulointerstitial inflammation consistent with acute tubulointerstitial nephritis was noted. Pulse followed by oral steroids was followed by recovery of kidney function. Proteinuria decreased after initiation of cyclosporine A.
