Yayın Anterior cruciate ligament reconstruction by using otogeneous hamstring tendons with home-based rehabilitation(Sage Journals, 2008) Üğütmen, Ender; Özkan, Korhan; Kılınçoğlu, V.; Özkan, Feyza Ünlü; Toker, S.; Eceviz, Engin; Altıntaş, FaikWe investigated patients undergoing arthroscopic anterior cruciate ligament (ACL) reconstruction using otogeneous hamstring tendons with a cross-pin technique to compare a post-operative home-based rehabilitation programme with a clinic-based programme. ACL reconstruction was performed on 104 patients (103 male) by the same surgeon. The mean age of the patients was 31.5 years (range 18 – 43 years) and the mean time interval between injury and operation was 34.3 months. Patients were randomly allocated to either a home-based (n = 52) or clinic-based rehabilitation programme (n = 52). Mean follow-up was 31.1 months (range 12 – 66 months). Patients underwent a series of examinations before and after surgery in order to evaluate functional recovery of their injured knee. The results demonstrated that using otogeneous hamstring tendons for ACL reconstruction was safe and produced satisfactory results. The study also demonstrated that a homebased rehabilitation programme was as effective as a clinic-based programme.Yayın Arthroscopic treatment at knee osteoarthritis(Medeniyet Üniversitesi, 2007) Üğütmen, Ender; Özkan, Korhan; Güven, Melih; Eren, AbdullahArthroscopic debridment was suggested as a useful and less invasive treatment alternative for patients with osteoarthritis not responding to conservative treatment or patients not necessiating radical surgical procedures. In our study, 130 patients who had undergone arthroscopic debridment for their degenerative osteoarthritis of knee between april 2001 and april 2006 in Göztepe education and training hospital second orthopaedics clinic were evaluated. 115 patients were female and mean age was 50.42 (SD 5.24). The mean followup time was 36.1 (26-48). When preoperative and postoperative (third week) satisfaction, Womac and VAS criterias were compared; the results were well with postoperative satisfaction scale was 1.28, Womac 36.16 and VAS 3.26. When cases were evaluated according to Patel and Aichroth criterias % 82.8 of patients were well at postoperative third months, % 75.6 of patients were well at postoperative 11.6 month but patients with satisfaction fell to % 62.5 at postoperative 36.1 month. The results of the arthroscopic debridment of knees with osteoarthritis will eventually detoriate with time. But it will gain time to patients with less symptoms until major surgical process. Eylül 2005-mayıs 2006 tarihleri arasında kalça ve diz eklem Replasman cerrahisi uygulanan 15 hastada (4 erkek, 11 kadın; Ort. Yaş 64; dağılım 42-85) ameliyat sonrası kullanılan ototransfüzyon seti ile postoperatif allojeneik kan transfüzyon gereksinimi değerlendirildi. Hiçbir hastaya preoperatif ve intraoperatif dönemde kan transfüzyonu yapılmadı. 15 hastanın 19 Alt ekstremitesine farklı endikasyonlarla artroplasti girişimi Uygulandı. Tüm hastalarda postoperatif hemovak dren yerine Ototransfüzyon seti kullanıldı ve cilt kapamasından 4,6 saat Sonra (dağılım 4-6 saat) ortalama 560 ml kan (dağılım 350- 800 ml) transfüze edildi. Tüm hastaların preoperatif, tansfüzyon öncesi ve sonrası, postoperatif 1. Gün, 3. Gün, 5. Gün, 1. Hafta, 3. Hafta ve 6. Hafta hemoglobin ve hematokrit değerleri Kaydedildi. Hiçbir hastada transfüzyona bağlı allerjik reaksiyon ve enfeksiyon gibi komplikasyonlar gelişmedi. Hastalardan sadece 1 (% 5.2) tanesine postoperatif birinci gün 1 ünite kan transfüzyonu yapıldı. Diğer hastalarda postoperatif dönemde allojeneik kan transfüzyon ihtiyacı olmadı. Transfüzyon uygulanan Hasta ile birlikte toplam 2 (% 10.4) hastaya postoperatif birinci hafta hemoglobin değer düşüklüğü nedeniyle oral demir Replasman tedavisi başlandı ve 3 hafta boyunca devam edildi. It was mainly observed unilaterally in adolescent females (F/M: 3/1–4/1) (Hay and Smith in The Foot 2:176–178, 1992). Bilateral occurrence and adjacent metatarsal involvement has been previously reported separately [Maresca et al. in Arthroscopy 12(1):103–108, 1996; Rafee et al. in Foot Ankle Surg 12:153–155, 2006]. The cases presented here involve two female patients aged 42 and 72 with bilateral Freiberg’s disease in both the second and third metatarsal heads. To our knowledge, this is the first report of such involvement of both the second and third metatarsal heads.Yayın Bilateral non-traumatic hip fractures in a heavy smoker COPD patient on inhaled steroids(SpringerLink, 2016) Sanel, Selim; Sezgin, Gülbüz; Sarıman, Nesrin; Üğütmen, Ender; Solakoğlu, CanBilateral proximal femoral fractures without trauma are very rare conditions. They have been reported in connection with osteoporosis, renal osteodystrophy, parathyroid disease, tumors, epileptic seizures, electroconvulsive therapy, and postirradiation. Method We present a case of a 75-year-old man with bilateral hip fractures. No trauma, neurological, endocrinological disorder, or malignancy was reported in his history. He had a background of chronic obstructive pulmonary disease (COPD) and had been taking inhaled steroids (budesonide) 800 µg per day for 10 years. He was a heavy smoker with a smoking history of 120 packs/year. His complaints had initially started as pain on the left hip and groin and then had progressed to the right in 10 days. Plain x-rays of the pelvis showed left femoral neck and right subtrochanteric femoral fractures. Fixation with proximal femoral nail of the right hip and partial arthroplasty of the left hip was performed on the following day after his admission. Pathological examination revealed osteoporosis in bone samples from both hips. Result COPD and osteoporosis have some common risk factors. Smoking, decreased exercise capacity, inhaled, or oral steroid therapy may increase osteoporosis and risk of bone fractures by decreasing bone mineral density. Non-traumatic femoral fractures may occur in patients on long-term inhaled steroid treatment for chronic airway diseases such as asthma and COPD. Conclusion History of COPD with corticosteroid use may be used as a diagnostic tool to identify patients having osteoporosis. Preventive measures can be performed by monitoring high-risk patients with bone mineral densitometry, WHO fracture risk assessment tool (FRAX tool), serum calcium, and vitamin D levels to prevent bone fractures. Treating those patients with the lowest effective dose of corticosteroids should be targeted.Yayın Bilateral simultaneous traumatic hip dislocation in opposite directions: a case report.(Europe PMC, 2009) Bilsel, Kerem; Alpan, Buğra; Üğütmen, Ender; Özkan, KorhanBilateral traumatic hip dislocation is rarely seen. A patient who presented with simultaneous dislocation of both hips in opposite directions is presented in this case report. A 42-year-old female patient who was injured in a motor vehicle accident as a passenger was brought to the emergency department and was diagnosed with posterior fracture dislocation of the right hip and anterior dislocation of the left hip. Both hips were reduced under general anaesthesia by closed manipulation, and reduction was confirmed with computerized tomography. No skeletal or skin traction was applied. Hip range of motion exercises were begun immediately. The patient was mobilized at the end of the sixth week and returned to daily activities without any complaint at the end of 16th week.Yayın Chondroprotective effect of salicylate and chloroquine in pyogenic septic arthritis(SpringerLink, 2008) Eren, Abdullah; Üğütmen, Ender; Özkan, Korhan; Turhan, Yalçın; Eceviz, Engin; Çilli, FeridunIntroduction This study aims to investigate the effectiveness of salicylate (Aspisol®; Bayer AG, Leverkusen, Germany) and chloroquine on preventing cartilage damage in septic arthritis. Methods A septic arthritis model was created by inoculating the knee joint of young rabbits (n=21) with Staphylococcus aureus. Some rabbits with inflicted septic arthritis did not undergo any treatment and served as the control group (n=7). The second group (n=7) was started on salicylate on day 2 to investigate the effects of salicylate on joint cartilage. The third group (n=7) was started on chloroquine on day 2 to investigate the effects of chloroquine on joint cartilage. All three groups underwent arthrotomy, drainage and synoviectomy on day 7 of the experiment. Animals were sacrificed on day 14 and the joint cartilages were extracted. Histopathology, determination of local prostaglandin (PGE)-like activity, and evaluation of hyaluronic acid loss were performed on all samples. Results The control group and the salicylate group were similar in the extent of joint damage. Salicylate did not prevent joint cartilage damage despite inhibiting PGE synthesis. Chloroquine, despite not inhibiting PGE synthesis, did prevent cartilage destruction. Conclusion If supported by larger-scale studies, chloroquine could be added to the antibiotic regimen in the treatment of septic arthritis to prevent cartilage damage.Yayın Distal tibial epiphyseal fracture (tillaux) and capsular interposition(APMA, 2009) Poyanlı, Oğuz; Ünay, Koray; Özkan, Korhan; Üğütmen, EnderArthroscopic-assisted reduction is one recommended technique for pediatric distal tibial epiphyseal fractures. A 15-year-old male patient with a triplane distal tibial epiphyseal fracture (Tillaux type) was scheduled for arthroscopic reduction and pinning, which failed to provide adequate fracture reduction. Open surgery was subsequently performed, which revealed a capsular interposition in the fracture line attributable to an anterior capsular tear of the ankle. The possibility of capsular interposition should always be taken into consideration when problems such as those described in this case presentation are encountered in such patients. This is precisely why direct visualization either through arthroscopy or open arthrotomy is sometimes necessary.Yayın Early tunnel enlargement after arthroscopic ACL reconstructions(Universa Press, 2007) Üğütmen, Ender; Özkan, Korhan; Güven, Melih; Şener, Nadir; Altıntaş, FaikThe aim of this study was to evaluate the tunnel enlargement phenomenon after ACL reconstructions performed with hamstrings tendons fixed using the cross pin technique. Sixty-two knees in 62 patients were followed for two years to evaluate the possible clinical implications of the femoral and tibial tunnel enlargements noted after ACL reconstruction. The reconstructions were done with hamstring tendons using the cross-pin technique. Evaluation was based on calculated clinical scores (IKDC and Lysholm knee scores) and quantified by KT-1000 measurements. Sagittal and coronal plane computed tomography and conventional radiography were performed 3 days after operation and were repeated after 3 and 6 weeks, 6, 12 and 24 months, to assess early tunnel enlargement. Although it seems that tunnel enlargement after ACL reconstruction has no impact on the clinical results, long-term implications and potential need for revision surgery must be assessed. In this study, tunnel enlargement was noted fairly early after operation and was thought to be related with drilling of the tunnels. A possible solution to this problem may be drilling the tunnels to a diameter 1 mm smaller than the measured graft diameter, then to enlarge the tunnels to the graft diameter with the appropriate tunnel dilator.Yayın The effects of proximal fibular osteotomy on the knee and ankle joints: a finite element analysis(Czech Society for Orthopaedics and Traumatology, Slovak Society for Orthopaedics and Traumatology and Publishing House Galen, 2021) Ünal, Ömer Kays; Dağtaş, Mirza Zafer; Demir, C.; Nacefov, Turan; Üğütmen, EnderPURPOSE OF THE STUDY In this study, our aim is to examine the effect of proximal fibular osteotomy on knee and ankle kinematics with finite element analysis method. MATERIAL AND METHODS One 62-year-old, female volunteer’s radiologic images were used for creating lower limb model. Osteotomized model (OM) which was created according to definition of PFO and non-osteotomized model (NOM) were created. To obtain a stress distribution comparison between the two models, 350 N of axial force was applied to the femoral heads of the models. RESULTS After PFO, the average contact pressure decreased 26.1% at the medial tibial cartilage and increased 42.4% at the lateral tibial cartilage. The Von Mises stresses decreased 57.1% at the femoral cartilage and decreased 79.1% at tibial cartilage. The stress on the tibial cartilage increased 44.6%, and stress on the talar cartilage increased 7.1% at the ankle joint. CONCLUSIONS FEA revealed that main loading at the knee joint shifted from medial tibial cartilage to the lateral tibial cartilage after PFO. Additionally, the stresses on each cartilage were redistributed across a wider and more peripheral area. Bu makalede asetabuluma yönelik çeşitli yöntemleri ele alarak kendi uygulamamızdan bahsetmeyi amaçladık.Yayın Evaluation of axillary nerve integrity and shoulder functions in patients who underwent lateral deltoid splitting approach(SelSistem, 2020) Ünal, Ömer Kays; Ateş, Miruna Florentina; Dağtaş, Mirza Zafer; Üğütmen, EnderAim: The most common complication of the Lateral deltoid splitting approach (LDSA), which is used in the shoulder area, especially for posterior extension fractures and other soft tissue pathologies, is axillary nerve injury. Determining the frequency of nerve injuries that may occur after LDSA is decisive for the applicability of this approach. Therefore, in our study, we aimed to evaluate the axillary nerve integrity and shoulder functions in patients who underwent LDSA. Methods: In this prospective cohort study, 55 patients who were operated with LDSA for proximal humerus fractures between February 2015 and July 2018 were evaluated. Among these patients, 35 were selected and included in the study. Six months later Electrophysiological tests (Electroneuromyelography – ENMG) and Constant Shoulder Score (CSS) were used for evaluation of each operated and non-operated shoulder. CSS difference between the operated and non-operated sides was graded as mild (11-20 point), moderate (21-30) and severe (>30). Results: Mean age of the group was 66 (9) years. Twenty-five patients were female and 10 were male. Mean follow-up time was 4 (1) years. Mean latencies of axillary nerve were 4.6 (1.8) msn, 3.7 (0.54) msn and mean amplitudes of axillary nerve were 6.6 (2.21) mV, 8.4 (2.80) mV in the operated and non-operated shoulders, respectively. There was no statically significant difference between the operated and non-operated sides according to latency and amplitude (latency P=0.25, amplitude P=0.16). Mean CSS of the patients were 28.7. CSS of 12 patients were severe (mean: 39.08), 18 patients, moderate (mean 25.4) and 5 patients, mild (mean 16). There was no statically significant correlation between CSS and axillary nerve latency / amplitude (P= 0.62, r=0.267 / P=0.98, r=-0.339). Fracture type and CSS showed a statically significant correlation (P=0.032, r= 0.829). Conclusion: This study revealed that LDSA provides wide and versatile fracture control without compromising the deltoid muscle functions and axillary nerve, especially in fractures extending to the posterior part of the proximal humerus.Yayın Femoral neck fractures treatment with straight stem endoprosthesis(Dergipark, 1995) Altıntaş, Faik; Konal, Adnan; Üğütmen, Ender; Türe, MehmetOcak 1991 ve Aralık 1993 tarihleri arasında SSK Göztepe Hastanesi II. Ort. ve Trav. Kliniğinde, femur boynu kırığı olan 23 erkek 26 kadın hastanın toplam 50 kalçasına modüler başlı düz saplı parsiyel endoprotez ameliyatı yapıldı. Hastaların ortalama yaşı 64.5 (38-92) idi. Ortalama 18 ay (2-34) takip edilen vakalar, postoperatif Harris'in fonksiyonel skorlama sistemine göre değerlendirildiğinde 38 (%86) inde tatminkar sonuç alındı. Case presentation A patient with bilateral comminuted, displaced distal fractures of the radius and bilateral fractures of the scaphoid was treated via internal fixation of the scaphoid fractures with Herbert screws and internal fixation of the distal radius fractures with locked volar plating. Conclusion Rigid internal fixation of distal radius and scaphoid fractures is mandatory to start early active rehabilitation of the wrist without the need for wrist immobilization with a plaster or external skeletal fixation.Yayın Incomplete triple pelvic osteotomy for the surgical treatment of dysplasia of the hip in adolescents and adults(The British Editorial Society of Bone & Joint Surgery, 2005) Eren, Abdullah; Ömeroğlu, H.; Güven, Melih; Üğütmen, Ender; Altıntaş, FaikWe treated 26 hips (24 consecutive patients) with residual dysplasia by a technique of incomplete triple pelvic osteotomy. The mean age of the patients was 21.6 years. The mean values for the pre-operative centre-edge angle of Wiberg, the refined centre-edge angle, the acetabular angle of Sharp, the modified acetabular angle and femoral head lateralisation were 7.7°, ?3.1°, 49.3°, 53.2° and 17.2 mm, respectively. After a mean follow-up of 3.3 years they were 27.0°, 13.0°, 38.9°, 44.3° and 15.9 mm, respectively (p < 0.05). The osteoarthritic grading changed adversely in one hip. The mean pre-operative and latest Harris hip scores were 74.9 and 93.0, respectively (p < 0.05). This technique provides a stable osteotomy with maintenance of the posterior column which allows early mobilisation and minimal internal fixation. The technique is not complex and requires minimal blood transfusion. The use of an image intensifier is not necessary and harvesting of a subcristal bone graft avoids post-operative complications at the donor site. Developmental dysplasia of the hip (DDH) may cause symptoms in later life. Surgical treatment by redirectional pelvic osteotomy is often recommended in adolescents and young adults.1 A technique of triple pelvic osteotomy was first described by LeCoeur,2 nearly 40 years ago, and since then several types of acetabular redirectional osteotomy including single, double and triple pelvic osteotomies and periacetabular and rotational acetabular procedures have been described.3–6 In this prospective study, we have evaluated a modification of triple pelvic osteotomy including an incomplete curvilinear iliac osteotomy.Yayın Intraoperative dislocation of the prosthetic femoral head into the pelvis during total hip arthroplasty(Universa Press, 2008) Özkan, Korhan; Üğütmen, Ender; Altıntaş, Faik; Eren, Abdullah; Mahiroğulları, MahirAn 86-year-old woman with severe osteoarthritis and severe pain in her left hip underwent total hip arthroplasty. Intraoperatively, the 22 mm femoral head impinged on the anterior rim of the acetabulum during a reduction maneuver, and disengagement of the femoral head from the neck occurred with migration of the head over the pelvic brim along the inner table of the pelvis. However, a femoral shaft fracture with an ipsilateral patella fracture has been, to the best of our knowledge, given only cursory attention in English-speaking literature, Also, vertical patella fractures seems to be rare and it is difficult to see in the routine roenthgenographies of AP and lateral views [4]. The mechanism of injury of Juvenil Tilloux is forced external rotation of the foot. The lateral physeal fragment is pulled by the anterior tibiofibular ligament. The aim of this article is to review our knowledge about this uncommon fracture which we have had a chance to treate.
