Yayın BILATERAL BR EAST RECONSTRUCTION(Medknow Publications & Media Pvt Ltd, 2008) Uluğ, Burçak Tümerdem; Kuran, IsmailThe increasing incidence of bilateral mastectomy for breast cancer and premalignant breast disease, with a significant improvement in methods available for mastectomy reconstruction has led to an increase in the indication for bilateral breast reconstruction. Seventeen patients who underwent bilateral breast reconstruction are evaluated in this study. The importance of preoperative planning to overcome the difficulties of bilateral breast reconstruction, the operation details of reconstruction with implant and the advantages of cohesive III implant are discussed. Two patients underwent reconstruction of one breast with transverse rectus abdominis myocutaneous flap and the other breast with implant. Two patients underwent reconstruction of one breast with latissimus dorsi myocutaneous flap and implant, the other breast only with implant. One patient had bilateral latissimus dorsi myocutaneous flap with implant reconstruction. Implants were used for bilateral breast reconstruction in the remaining. The results were evaluated in 85% as good and minor revisions were needed in 20%. Simultaneous bilateral breast reconstruction presents unique challenges and technically, it is much more difficult than a unilateral reconstruction. However, one of its advantages is that there is not any ptotic contralateral breast which frequently causes difficulty to obtain symmetry with. If the patient does not accept breast reconstruction with autogenous tissue and had sufficient soft tissue coverage, bilateral breast reconstruction with implant is an ideal option with its shorter operation and healing time.Yayın Does Hyperbaric Oxygen Administration Before or After Irradiation Decrease Side Effects of Irradiation on Implant Sites?(LIPPINCOTT WILLIAMS & WILKINS, 2011) Tumerdem-Ulug, Burcak; Kuran, Ismail; Ozden, Burcu C.; Mete, Ozgur; Kemikler, Gonul; Aktas, Samil; Calik, BurcuBackground: One of the main limitations of implant-based breast reconstruction is the high rate of complications such as capsular contracture and poor aesthetic outcome, due to adjuvant radiotherapy. Hyperbaric oxygen treatment (HBOT) has been used to assist wound healing in the prevention and treatment of the side effects of irradiation. We aimed to investigate this effect of HBOT on the capsule reaction and skin, applied before and after irradiation, following the placement of an implant under the dorsal skin of the rat. Methods: Fifteen Wistar rats were randomly divided into 3 groups. A 18-mL smooth testicular implant was introduced into a subcutaneous pocket located on the dorsum of each rat. A single dose of 17-Gy irradiation was given to the implanted area of each rat at the third week. HBOT which lasted 3 weeks was administered to group I before irradiation, group II after irradiation. The control group did not receive HBOT. All of the rats were killed at the ninth week (6 weeks after irradiation). The dorsal skin with the capsule overlying the implant were excised for histopathological processing. The thickness of the capsule reaction of each group was evaluated statistically. Results: Our histopathological examination revealed changes due to radiation in the control group. Many of these findings were found to be more subtle in group I and nearly absent in the group II. There was not any statistical difference between the thickness of the capsule reactions of the control group and group I, or group I and group II, but the capsular thickness of the control group was statistically higher than group II. Conclusion: It can be predicted that the use of HBOT following irradiation is an effective tool to reduce the capsule reaction of the implanted area and the tissue damage seen in radiodermatitis.Yayın THE IMPORTANCE OF THE CHIN DEFORMITY IN RHINOPLASTY CASES(Medknow Publications & Media Pvt Ltd, 2009) Tümerdem Uluğ, B.; Kuran, IsmailPatients requesting for rhinoplasty may have mild to moderate chin deformities. Augmentation mentoplasty with an alloplastic implant is a minor procedure and can be combined with rhinoplasty. We performed rhinoplasty combined with augmentation mentoplasty in 8 patients who had normal occlusion. Two-piece porous polyethylene implant was used for the chin augmentation. No major complication was observed. One patient had revisional surgery to change a larger implant. The numbness over the chin resolved in a couple of weeks. The aesthetic results of the combined procedure especially on the lateral view were quite satisfactory. Rhinoplasty combined with augmentation mentoplasty provides a better balanced face. Therefore a detailed preoperative facial analysis should be done for the patient who refers for rhinoplasty.Yayın Nasal Reconstruction Based on Subunit Principle Combined With Turn-Over Island Nasal Skin Flap for Nasal Lining Restoration(LIPPINCOTT WILLIAMS & WILKINS, 2008) Ulug, Burcak Tumerden; Kuran, IsmailThe reconstructive plan of full-thickness nasal defects should include recreation of internal nasal lining, skeletal and cartilaginous Support and external cover. We performed nasal reconstruction combined with turn-over island nasal skin flap for nasal lining reconstruction in 12 patients who underwent full-thickness resection of nasal area because of skin cancer. The flap filled the nasal lining defect of the alar area and provided a good support for skeletal reconstruction. The external skin coverage, which was enlarged according to the Subunit principle, was restored by the forehead skin flap at the same operation.
